All volunteers participating in Dallastown Cougar Softball Organization need to create a user account on our registration page and submit required clearance and background check documents.
To create an account, click on the registration tab, then click volunteer opportunities. By creating a user account it doesn't commit you to volunteering or guarantee a coaching position. It does give us the a list of individuals and their contact information to begin the coaching selection process.
The forms, along with instructions, can be found in the links below. In order to participate in any team activities, all volunteers must submit, and have approved, the following forms:
Required Annually:
- All head and assistant coaches CDC Heads Up Concussion Certificate: headsup.cdc.gov
Required Every 5 years:- PA State Police Criminal Record Clearance: https://epatch.state.pa.us/
- PA Child Abuse History Clearance: https://www.compass.state.pa.us/cwis/public/home
- Proof of Fingerprinting: https://uenroll.identogo.com (use service code: 1KG6ZJ) or if you've been a PA resident for 10 or more years use the Exemption Form.
- Dallastown School District Mandated Reporter Form (https://www.dallastown.net/departments/human_resources/clearances/clearances_-_volunteers)
Submit all documents together in person or electronically by email to DCA's safety coordinator at the following email address
[email protected]. For in-person submission, please contact DCA's safety coordinator via email to arrange a place to meet so clearances can be handed in. If submitting clearances by email, do not send photographs of the clearance documents. Submit either as an Adobe .pdf file or a .docx word document file.
All forms will need to be submitted and approved before any volunteer can participate in team activities.
